May 22, 1968: The USS Scorpion disappears SW of the Azores after conducting a mission to observe Soviet naval activity.  In October her remains are located on the seabed by the research ship Mizar

No official cause has ever been determined for the accident that killed 99 crewmen; the US Navy has considered several theories of the “unexplained catastrophic event” including a torpedo malfunction.
